This page contains a list of forms - many of which can be filled in online and either submitted or printed and sent in.

Please choose the appropriate registration package below.  Please be sure to include your payment along with the registration package. Your child is considered enrolled when all the documents and payments are received at the office.

For new families, please be sure to include a copy of your child’s birth certificate along with a copy of their immunization record and this Immunization Form with your registration package.

  • New Family Kindergarten Only: New families should contact the school directly to obtain a registration package and arrange to meet with the Principal. Complete this form if you are registering one or more children in JK or SK only and you did NOT have children enrolled at OCS during the 2011/12 school year.   The online form will do all the calculations for you.
  • New Family Grades 1 to 8: New families should contact the school directly to obtain a registration package and arrange to meet with the Principal. Complete this form if you are registering at least one child in grades 1 to 8, and you did NOT have children enrolled at OCS during the 2011/12 school year.

Returning families that have new children attending OCS will need to fill out this Immunization Form and send a copy of your child’s birth certificate and immunization record into the office.

  • Returning Family Kindergarten Only: Complete this form if you are enrolling one or more children in JK or SK only and your family was enrolled at OCS during the 2011/12 school year.   The online form will do all the calculations for you.
  • Returning Family Grades 1-8: Complete this form if you are enrolling at least one child in grades 1 to 8, and you had children enrolled at OCS during the 2011/12  school year. The online form will do all the calculations for you.
