This page contains a list of forms many of which can be filled in online and either submitted or printed and sent in.
Please include the following items to complete your registration. They are available below:

- Consent for School Trips and Photo Release Form
- Pre-Authorized Debit Registration Form or 10 cheques
- Bus Registration Form (if applicable)
- Health and Medical Form
- Class Trip Consent and Photo Release Form

- Non-refundable tuition deposit cheque (new families only)
- Progress Report from previous school (new families only)
- Copy of Birth Certificate (new students only)

- TRIP Registration form (Optional: new families or those who have not yet registered for this)
Membership Application
Membership Application Form - Print and complete this form if you wish to become a member of OCS.
Student Registration Form
Fill in one of these forms per student that is being registered at OCS for the first time.
- Student Registration Form - This form is for New Families only. Please print and send in with your Registration.
- Kindergarten Registration Form: This form is to be filled in if the student is entering Kindergarten.
Pledge Forms
Returning families for the 2010-11 school year can print and complete the appropriate form and return it to the school with tuition payment. New families should contact the school directly to obtain an admission package and arrange to meet with the Principal.
2010-2011 Pledge Forms:
- Kindergarten: Complete this form if you are enrolling one or more children in JK or SK only. Pledge Form Kindergarten 2010-11
- Returning Family, Grade 1 - 8: Complete this form if you are enrolling at least one child in grades 1 to 8, and you had children enrolled at OCS during the 2009/10 school year. OCS Returning Family Grade 1-8, 2010-11
- New Family: Complete this form if you are enrolling at least one child in grades 1 to 8, and you did NOT have children enrolled at OCS during the 2009/10 school year. Pledge Form New Family 2010-11
Pre-Authorized Debit Registration Form
Pre-Authorized Debit Registration Form - This form can be filled in online.
Bus Registration Form
Bus Registration Form 2010/11 - This can be submitted completely online - no signature necessary.
Health and Medical Form
Health and Medical Form - This can be submitted completely online - no signature necessary.
Consent Forms
Class Trip Consent and Photo release - Please complete this form ONLINE and return to the school office with your Tuition forms.
TNT Volunteer Commitment Form
TNT Volunteer Commitment Form - Please complete this form ONLINE and return to the school office with your Tuition forms. Remember to attach your Volunteer Assessment cheque ($500) dated May 31 of the next year.

Children First Grants
Important Announcement from Children First: School Choice Trust:
In an effort to meet the financial needs of our current grant recipients, Children First: School Choice Trust will not be offering new grants for the 2010-2011 school year. This means that we will not be accepting new applications in Ontario during our traditional January-March application drive.
Children First remains committed to our existing grant recipients and our financial support for these students will remain unchanged for the duration of the 2009-2010 school year.
Families who are eligible to renew their grants for the 2010-2011 school year will receive their renewal kits by mail in early March.
We look forward to continuing our work with Ontario’s independent school community to help families in financial need afford the school of their choice.
